Media Release

I consent and grant permission to New Hope Christian Missionary Baptist Church (and/or its authorized representatives, contractors, subsidiaries, licensees and designees) the use of my image, likeness and/or recorded voice as captured through photographs, electronic (video) images, and sound/video recordings without payment or any other consideration. I understand that I may become recognizable through such use. I warrant that I have the right and authority to grant this consent and further grant New Hope Christian Missionary Baptist Church all rights and titles to submitted original and reproduced copies of my image, likeness and/or recorded voice.

I understand that my image may be edited, copied and/or exhibited and I waive the right to inspect or approve the finished product wherein my image, likeness and/or recorded voice appears.
Additionally, I waive any right to royalties or other compensation arising from or related to the use of my image, likeness and/or recorded voice. I also agree to indemnify, hold harmless and release New Hope Christian Missionary Baptist Church from all claims or demand resulting from this consent.

Photographs, audio and/or video recordings may be used for (but not limited to) the following purposes via any source including print, broadcast, and/or electronic media:

Church services, brochures and/or newsletters
Church-related marketing, communications and/or advertising
Church website and/or its social media outlets

I understand that I am free to address any specific question(s) regarding this consent and release by submitting those questions in writing prior to signing.

This consent will be effective the date of signing unless I revoke it in writing. I acknowledge that I have read the consent/release information contained above and I fully understand its contents.
